Copay cards, free medicine programs and charity funds for 210 rare disease treatments, sorted by your insurance, or by what is open to you if you have none.

We read the support program for each of 200 medicines on the drugmaker's own website, and we track 266 charity funds covering 128 conditions, 142 of which were open when we last checked on September 24, 2026. Trial Friend takes no money from any of these programs or drugmakers.

Which help fits your insurance

Private insurance

Start with the drugmaker's copay card; many bring the cost down to a small fixed amount, up to a yearly maximum. Ask your plan whether copay card payments count toward your deductible, since some plans don't count them. If your plan denies the drug, ask the drugmaker's support line about bridge supplies and appeal help.

Medicare

Drugmaker copay cards can't be used with Medicare (HHS Office of Inspector General). What can help: in 2026, what you pay out of pocket for Part D drugs is capped at $2,100 for the year (CMS), and the Medicare Prescription Payment Plan lets you spread those costs across the year at no charge. Charity funds are the main outside help, and some drugmaker free medicine programs accept Medicare patients who meet income limits. Medicare also lists programs that lower costs, such as Extra Help. Medicaid, TRICARE or VA

Copay cards are excluded here too. Medicaid copays are usually small; if a drug isn't covered, the drugmaker's support line can explain the options. Some TotalAssist funds accept Medicare, Medicaid or TRICARE.

No insurance

Free medicine programs are built for you, usually with income limits. Most charity funds require some insurance, so they may not apply. The drugmaker's support line can also check whether you qualify for coverage you don't have yet.

Before you call

Call the drugmaker's support line first. Their case managers check your benefits, find the right program and often handle the forms with your doctor.
Have your insurance card, your prescription details and proof of household income ready. Free medicine programs usually ask for income.
Most programs must be renewed every year, and many copay cards reset in January.
If a charity fund is closed, sign up on the foundation's page to be told when it reopens. Funds for the same condition at another foundation may still be open.
Specialty pharmacies and hospital financial counselors know these programs well. Ask them too.

Common questions

Can I use a drugmaker copay card if I have Medicare?

No. Federal rules treat drugmaker copay coupons used for drugs paid by Medicare, Medicaid or TRICARE as a possible kickback, so copay cards say government insurance is excluded. People on Medicare usually turn to independent charity funds, a drugmaker's free medicine program if it accepts Medicare patients, and Medicare's own help: in 2026, Part D out-of-pocket costs are capped at $2,100 for the year.

What's the difference between a copay card and a free medicine program?

A copay card lowers what you pay at the pharmacy when private insurance covers the drug. A free medicine program (often called a patient assistance program) gives the drug at no cost to people without coverage, or with coverage that doesn't help enough, and usually has income limits.

Why does a charity fund say closed?

Charity funds such as HealthWell and TotalAssist run on donations for each disease. When a fund's money runs out for the year it closes to new applicants, then reopens when new donations arrive. Most foundations let you sign up to be told when a fund reopens.

Do I need insurance to get help from a charity fund?

Usually, yes. The disease funds listed here generally help insured people with out-of-pocket costs such as copays and premiums. Some TotalAssist funds require Medicare, Medicaid or TRICARE. If you have no insurance, a drugmaker's free medicine program is usually the place to start.

How current is this information?

Each drugmaker program shows the date we checked it on the drugmaker's own pages, and each charity fund shows its open or closed status as the foundation listed it on the date shown. The oldest check on this page is September 24, 2026. Programs change, so confirm with the program before you rely on it.

Drugmaker programs come only from the drugmaker's own brand, program or foundation pages, never from aggregator sites. Where a page doesn't say who qualifies, we say so instead of guessing. Charity fund status is copied from each foundation's fund list on the date shown. We re-check programs at least every 90 days.
