Trutakna is the IgA nephropathy drug you give yourself once a week, at home, with a pen-like autoinjector. Nothing about it requires a REMS, a pregnancy test or a monthly lab before the pharmacy ships. The first fill waits on 3 ordinary things instead, your vaccines, an insurance approval and a call from the specialty pharmacy. This page lays out those first 3 months from the FDA label, Vera Therapeutics' own support program and one insurer's written rules.

What has to happen before the first bottle ships

Trutakna has no REMS, so the first fill waits on your vaccines, an enrollment form, an insurance approval and the pharmacy's call. These are the 5 things in the way, in order.

A check for active infection

Trutakna lowers the antibodies your immune system makes, and the label says this may raise the risk of infection. It tells prescribers to check for an active infection before the first dose. If you have one, the label says to delay Trutakna until it clears or is treated.FDA label

Vaccines brought up to date, live ones at least 30 days ahead

The label asks that all vaccines due for your age be completed before the first dose. Live vaccines are not recommended within 30 days before starting or during treatment. Vera's prescriber site adds that no specific vaccine is required to start.Vera prescriber site

The Tru Support enrollment form, signed by you and your doctor

Tru Support says patients cannot enroll themselves in the program. Your doctor's office submits a combined enrollment and prescription form online through CoverMyMeds or by fax. You sign the consent pages, and the office attaches the biopsy, protein and kidney function results. A Tru Support Case Manager then makes a welcome call.Enrollment form

The insurance approval

Vera's checklist for offices says each plan sets its own rules, and it lists the items plans commonly request. They include the biopsy, a urine protein result, and past treatments such as an ACE inhibitor, an ARB, a steroid or an SGLT2 inhibitor. A Tru Support Case Manager helps the office work out what your plan wants.PA checklist

The specialty pharmacy's call

Trutakna is only available through select specialty pharmacies that ship to your home. Vera's form names PANTHERx Rare Pharmacy and CVS Specialty Pharmacy. Tru Support says the pharmacy must speak with you directly before it can ship, so answer calls from unfamiliar numbers that week.Tru Support

The days that matter

Counted from the first injection, with the vaccine window before it, a weekly shot, and a new carton every 28 days. Type your first-dose date and the tiles become real dates you can add to your calendar.

The tiles below count days from a first dose. Enter the date of yours, or the date you expect, and each tile becomes a real date on your calendar.

The date stays on your device. It is not sent to Trial Friend or anyone else. The calendar file works with Apple, Google and Outlook calendars.
  1. Vaccines reviewed, live ones done

The label asks for age-appropriate vaccines to be completed before day 1. The label does not recommend live vaccines in the 30 days before the first dose.

  2. Enrollment form and prior authorization

The office submits the Tru Support form with your signed consents, and the prior authorization goes to your plan. If a commercial plan is slow, the Tru Support bridge program may supply a 28-day carton while coverage is pursued.

  3. The specialty pharmacy calls

It arranges delivery of the carton to your home. Tru Support says the pharmacy must speak with you directly before it can ship the first one.

  4. First injection, 150mg

Take one autoinjector out of the refrigerator and let it sit 15 to 30 minutes first. Inject into the belly or the front of the thigh, holding it down to a slow count of 15. A carton holds 4 autoinjectors, a 28-day supply.

  5. Same day each week

One autoinjector every 7 days, in a different spot from the last one, as the label directs. If a dose is missed, the new weekly day starts from the late dose.

  6. Same day each week, number 2

  7. Same day each week, number 3

  8. Same day each week, number 4

  9. Refill, then every 28 days

A carton of 4 autoinjectors runs out here. Vera's prescription form writes it as 1 carton per 28 days with 11 refills, which covers about a year.

  15. Same day each week, number 9

  16. Same day each week, number 10

  17. Same day each week, number 11

  18. Three months in

The label sets no scheduled blood test for Trutakna. The main trial measured urine protein from a 24-hour collection at week 36, about 8 months in. When to recheck it is the nephrologist's call.

The label, revision by revision

The FDA lists which sections changed in each revision of a label. These are the revisions the current Trutakna label records, dated by the FDA, with a plain note on what each one meant for a patient. Older references and handouts can lag these by months.

Current label dated August 5, 2026, read from the FDA's label database today. FDA label

  1. July 2026

How the label says to take it

The parts of the prescribing information and the patient leaflet that come up in the first 3 months, in plain words.

  • Once a week, 150mg, under the skin

The label dose is 150mg once weekly, one single-dose autoinjector each time. Inject into the belly at least 2 inches from the navel, or the front of the thigh. Use a different spot each time.FDA label

  • Warm it up, but never heat it

Let the autoinjector sit at room temperature for 15 to 30 minutes before the shot. The Instructions for Use say warming may make the shot more comfortable. Never microwave it or use any other heat source.FDA label

  • Wait for the blue window

Hold the autoinjector firmly against the skin and count slowly to 15. The viewing window should be completely blue before you lift it. If the window is not fully blue, or more than a few drops are on the skin, call your doctor about the dose.FDA label

  • If you miss a dose

The label says to inject the missed dose as soon as possible. Your weekly schedule then restarts 1 week from that late dose, so your injection day may move.FDA label

  • Refrigerator, original carton

Store it at 36 to 46°F (2 to 8°C) in its carton to protect it from light. One autoinjector may stay at room temperature up to 77°F (25°C) for a single period of 3 days. After that it never goes back in the refrigerator and is thrown away if unused. Never freeze it, shake it or leave it near heat.FDA label

  • Travel

Vera's patient site says to keep the autoinjector between 36 and 46°F (2 and 8°C) while traveling, away from light. Medicines may need extra airport screening, and Vera points to the TSA Cares page.Trutakna patient site

  • Vaccines during treatment

Live vaccines are not recommended at any point during treatment. The label also says Trutakna may weaken the response to other vaccines. The Patient Information asks you to tell your healthcare provider about any vaccine you recently had or have scheduled.FDA label

  • Steroids and other immune medicines

The label says using Trutakna with medicines that affect the immune system, including steroid pills, may raise the risk of infection. Using it with other immune-modulating drugs has not been studied. Tell the nephrologist and pharmacist about every medicine and supplement you take.FDA label

  • Pregnancy and breastfeeding

The label says there are not enough data to know the risk in pregnancy. Based on how it works, it may weaken the immune system of a baby exposed in the womb. If you become pregnant during treatment, the label asks you or your doctor to call Vera at 1-833-633-8372. The label also says it is not known whether Trutakna passes into breast milk.FDA label

  • Throwing away used autoinjectors

Put each used autoinjector straight into an FDA-cleared sharps container. A heavy plastic household container with a tight, puncture-resistant lid can stand in. Follow your community's rules for the full container, and never recycle it.FDA label

What the label says to act on

The symptoms the label singles out, and what it tells patients and prescribers to do about each. If any of these happen, the label's instruction is the one to follow, with your care team.

Signs of infection

The Patient Information lists fever, flu-like symptoms, chills, aches and pains, feeling tired, warm, red or painful skin, and cough. It says to tell your healthcare provider right away. The label lets the prescriber pause Trutakna until a serious infection is under control.FDA label

Infections in the trial

In ORIGIN 3, infections were reported in 32% of patients on Trutakna against 28% on placebo. Upper respiratory infections such as colds were the most common, at 12% against 9%. Most side effects were mild or moderate and cleared without stopping treatment.FDA label

Reactions where you inject

Injection site reactions occurred in 19% of patients on Trutakna against 2% on placebo, and redness at the site in 6% against 1%. The Patient Information describes redness, swelling, itching or irritation. The label says to report any severe reaction.FDA label

Signs of an allergic reaction

Breathing problems, swelling of the face, eyes, lips, tongue or throat, dizziness or lightheadedness, itching, hives or a skin rash. The Patient Information says to get emergency help for any of these.FDA label

Why the pharmacy or the insurer says no, and what fixes it

Most first-fill delays are one of these. Knowing which one tells you who to call.

  • The prescription went to a pharmacy that does not carry itTrutakna ships only from select specialty pharmacies. Vera's enrollment form offers PANTHERx Rare Pharmacy and CVS Specialty Pharmacy, unless your plan requires another. The office resends the prescription, and this is a routing problem, not a denial.Enrollment form
  • The pharmacy has not reached youTru Support says the specialty pharmacy must speak with you directly before it ships. A missed call from the pharmacy can hold up the first box. Call the pharmacy back, or Tru Support at 800-729-8591 if you do not know which pharmacy has it.Tru Support
  • Tru Support has no record of youPatients cannot enroll themselves, so the request has to come from the doctor. Your doctor's office submits the form through CoverMyMeds or by fax to 800-903-0428, after you sign the consent pages. Ask the office whether it went in.Tru Support for offices
  • Prior authorization pending or deniedVera's checklist says plans commonly ask for the biopsy, a urine protein result and past medicines such as an ACE inhibitor or ARB. Vera's office checklist suggests a letter of medical necessity, because plans take time to set rules after an approval. For commercial insurance, the bridge program may supply a 28-day carton while coverage is pursued.PA checklist
  • A copay you cannot payCommercial insurance may qualify for the Tru Support copay program, as low as $0 up to a yearly maximum Vera does not publish. Medicare, Medicare Advantage, Medicaid and TRICARE are excluded by its terms. Uninsured or underinsured patients who meet an income limit may get it free through Vera's Patient Assistance Program. What one insurer asks for, as an example

Aetna Better Health (Medicaid plans in Illinois, New Jersey, Maryland, and the Pennsylvania and Florida children's plans) prior authorization policy, September 30, 2026

  1. An adult with primary IgA nephropathy confirmed by kidney biopsy
  2. Protein in the urine of 0.5g a day or more, or a urine protein-to-creatinine ratio of 0.5g/g or more, measured within 3 months before starting
  3. At least 3 months on a stable, maximally tolerated ACE inhibitor or ARB, unless one cannot be taken
  4. Prescribed by or in consultation with a nephrologist

Approval length. A year at a time, renewed when the records show benefit, such as lower or stable urine protein or stable kidney function, with a limit of 4 autoinjectors per 28 days.

This is a Medicaid policy, and Vera's copay, bridge and free-drug programs exclude Medicaid by their terms. As of November 7, 2026, we found no public commercial policy for Trutakna from Express Scripts, FEP Blue, Aetna or Cigna. It is here as an example of what a plan asks for, not as a rule. Five questions for the pharmacist or the support line

  1. Which specialty pharmacy are you, and what number will your calls come from?
  2. How will the carton be packed to stay cold, and what should I do if it arrives warm?
  3. When will each 28-day carton ship, and how early do you call before it?
  4. I take these other medicines, including any steroid. Does anything here raise my infection risk?
  5. Where can I get a sharps container, and is one covered by my plan?

Questions people ask in the first weeks

How long does it take to get Trutakna after it is prescribed?

There is no fixed number of days. The enrollment form, the prior authorization and the pharmacy's call each take their own time. The label also does not recommend live vaccines in the 30 days before the first dose. Vera's bridge program exists for commercial plans that are slow to decide.

Do I need vaccines before Trutakna?

The label asks that all vaccines due for your age be completed before the first dose. Live vaccines are not recommended within 30 days before starting or during treatment. Vera's prescriber site says no specific vaccine is required to start.

Can I pick up Trutakna at my regular pharmacy?

No. Vera says Trutakna is only available through select specialty pharmacies that ship to your home. Its enrollment form names PANTHERx Rare Pharmacy and CVS Specialty Pharmacy.

What if I miss a dose of Trutakna?

The label says to inject the missed dose as soon as possible. The once-weekly schedule then restarts 1 week from that dose.

Can Trutakna be left out of the refrigerator?

One autoinjector may be kept at room temperature up to 77°F (25°C) for a single period of 3 days. After that it cannot go back in the refrigerator and is thrown away if unused.

Does Trutakna require blood tests?

The label sets no scheduled blood test and has no REMS. It asks prescribers to check for active infection before starting and to watch for infection during treatment. How often to recheck urine protein and kidney function is the nephrologist's call.

Is Trutakna fully approved?

Not yet. The FDA granted accelerated approval on July 7, 2026, based on lower urine protein. Vera says it filed for full approval on November 3, 2026, with a decision expected in mid-2027.
