Prescribing companion
Trutakna (atacicept)Dosing, vaccine timing, infection monitoring and prior authorization criteria for IgA nephropathyFor prescribers and their offices treating IgA nephropathy: what the pharmacy needs before it ships, what the prior authorization wants documented, the titration and the monitoring the label requires.
Dispensing gate
No REMS and no lab gate; assess for active infection, complete age-appropriate vaccines with any live vaccine more than 30 days before day 1, submit the Tru Support enrollment and prescription form, and the specialty pharmacy must reach the patient before it ships.
Example PA threshold
Aetna Better Health Medicaid example: biopsy-confirmed primary IgAN, adult, proteinuria 0.5g a day or UPCR 0.5g/g or higher within 3 months, 3 months of maximally tolerated ACEi or ARB unless intolerant, nephrologist.
Monitoring
No scheduled lab in the label; infection assessment before starting and vigilance throughout, interruption to consider for serious infection, no live vaccines during treatment.
Support line
Tru Support, 800-729-8591, Monday to Friday 8 AM to 8 PM Eastern, enrollment by CoverMyMeds or fax 800-903-0428, copay to $0 for commercial plans with an annual maximum, 28-day bridge supply, Patient Assistance Program, Nurse Educators.
Give your patient their plan
A one-page handout for starting Trutakna, in plain language: what the first fill waits on, the first dates, who will call and the symptoms to report, with blank lines for the start date and your office's number. The link opens the patient's full first 90 days page, where their dates become calendar entries.
trialfriend.com/p/trutaknaBefore the first fill: REMS, labs and enrollment
The steps that the pharmacy, the REMS or the drugmaker's program will wait on, in the order they usually need to happen. A first fill of a specialty drug most often stalls on one of these rather than on the plan.
| Step | What it requires | Source |
|---|---|---|
| Screen for active infection | The label says to assess for active infection before initiating and to delay Trutakna until it resolves or is adequately treated. Patients with chronic or recurrent infection may be at higher risk of serious infection. | FDA label |
| Immunizations | Complete all age-appropriate immunizations according to current guidelines before initiating. Live vaccines are not recommended within 30 days before initiation or during treatment, as the safety of coadministration has not been established. | FDA label |
| Enroll in Tru Support | The Patient Enrollment and Prescription Form doubles as the prescription. Submit it through CoverMyMeds or by fax to 800-903-0428 with the biopsy, recent UPCR or proteinuria, eGFR and the medication history. The patient signs pages 4 and 5; patients cannot self-enroll. | Enrollment form |
| Write the prescription | Vera's form prints it as 150mg/mL single-dose autoinjector, 150mg SC once weekly, dispense 1 carton (4 autoinjectors, a 28-day supply), 11 refills. Preferred pharmacy is PANTHERx Rare or CVS Specialty unless the payer mandates another. | Enrollment form |
| Prior authorization | Submit by ePA through CoverMyMeds, directly to the plan, or let the specialty pharmacy initiate it. Vera's checklist notes plans may take time to set coverage after approval and suggests a letter of medical necessity; templates for medical necessity and appeal letters are on Tru Support. | PA checklist |
| Bridge and the pharmacy call | For commercially insured patients facing a coverage delay, the bridge program may supply a 28-day carton; an additional 28-day refill is subject to limitations and requires that coverage is actively pursued. The specialty pharmacy must speak with the patient directly before it ships. | Tru Support |
Prior authorization criteria and what to document
One public policy as the example: Aetna Better Health (Medicaid plans in Illinois, New Jersey, Maryland, and the Pennsylvania and Florida children's plans), September 30, 2026. Approval a year at a time, renewed when the records show benefit, such as lower or stable urine protein or stable kidney function, with a limit of 4 autoinjectors per 28 days. This is a Medicaid policy, and Vera's copay, bridge and free-drug programs exclude Medicaid by their terms. As of November 7, 2026, we found no public commercial policy for Trutakna from Express Scripts, FEP Blue, Aetna or Cigna. It is here as an example of what a plan asks for, not as a rule.
| Element | What the policy asks | In the chart | Source |
|---|---|---|---|
| Diagnosis | Primary IgA nephropathy confirmed by kidney biopsy, adult member | Pathology report with date | Aetna Better Health policy |
| Proteinuria | 0.5g a day or more, or UPCR 0.5g/g or higher, obtained within 3 months before initiation | Lab report or chart note with date and method | Aetna Better Health policy |
| RAS blockade | Stable, maximally tolerated ACE inhibitor or ARB for at least 3 months, or an intolerance or contraindication | Agent, dose, start date, and the reason if below the labeled maximum or not used | Aetna Better Health policy |
| Prescriber | Prescribed by or in consultation with a nephrologist | Specialty on the request, or the consult note | Aetna Better Health policy |
| Renewal | Benefit from therapy, such as lower or stable proteinuria or UPCR, or stable kidney function | Follow-up UPCR or proteinuria and eGFR with dates | Aetna Better Health policy |
| Quantity | 4 autoinjectors per 28 days; 12-month initial and renewal approvals | Prescription written as 1 carton per 28 days | Aetna Better Health policy |
| Other plans | Vera's checklist lists commonly requested items: biopsy, UPCR or 24-hour proteinuria, prior ACEi or ARB, glucocorticoid and SGLT2 inhibitor use with failure or intolerance, and supportive care measures | Medication list with dates; blood pressure and lifestyle plan | PA checklist |

Monitoring the Trutakna label requires
| When | What | What the label says to do with it | Source |
|---|---|---|---|
| Before day 1 | Active infection; immunization status | Delay initiation until an active infection resolves or is treated; complete age-appropriate vaccines, any live vaccine more than 30 days before | FDA label |
| Any time | Infection: fever, flu-like symptoms, chills, aches, fatigue, warm, red or painful skin, cough | Consider interrupting Trutakna until a serious infection is controlled; infections 32% against 28% on placebo in ORIGIN 3, upper respiratory infection 12% against 9% | FDA label |
| Any time | Hypersensitivity or systemic administration reaction | Counsel patients to seek immediate care; serious hypersensitivity to atacicept-vymj or excipients is the only contraindication | FDA label |
| Any time | Local administration reactions | 30% against 5%; injection site reaction 19% against 2%, erythema 6% against 1%; mostly mild or moderate and resolved without interruption | FDA label |
| Pharmacodynamics | Serum Gd-IgA1, IgA, IgG and IgM | Fell within 4 weeks and stayed down through week 36: 68%, 64%, 36% and 75% from baseline. The label sets no immunoglobulin monitoring schedule | FDA label |
| Immunogenicity | Anti-drug antibodies | 43.2% (60 of 139) developed ADA through week 36, with no clinically significant effect on PK, PD, safety or effectiveness | FDA label |
Trutakna drug interactions and contraindications
| Label category | Agents and what the label says | Source |
|---|---|---|
| No section 7 | The label has no drug interactions section and no metabolism studies; atacicept-vymj is a fusion protein cleared by catabolism | FDA label |
| Immune-modulating drugs | Concomitant use with other immune-modulating therapies has not been evaluated; use with drugs that affect the immune system, including systemic corticosteroids, may increase infection risk | FDA label |
| Vaccines | Live vaccines not recommended within 30 days before initiation or during treatment; response to other vaccines may be reduced | FDA label |
| Pregnancy and lactation | Data insufficient to evaluate risk; may cause immunosuppression in the in utero exposed infant; report exposure to 1-833-633-8372; no data on milk transfer | FDA label |
| Postmarketing | The approval letter requires a worldwide pregnancy exposure study and a lactation study, and a confirmatory trial with a final report due July 2027 | FDA letter |
Why a first fill stalls, and the fix
- The prescription went to a pharmacy that does not carry itTrutakna ships only from select specialty pharmacies. Vera's enrollment form offers PANTHERx Rare Pharmacy and CVS Specialty Pharmacy, unless your plan requires another. The office resends the prescription, and this is a routing problem, not a denial.
- The pharmacy has not reached youTru Support says the specialty pharmacy must speak with you directly before it ships. A missed call from the pharmacy can hold up the first box. Call the pharmacy back, or Tru Support at 800-729-8591 if you do not know which pharmacy has it.
- Tru Support has no record of youPatients cannot enroll themselves, so the request has to come from the doctor. Your doctor's office submits the form through CoverMyMeds or by fax to 800-903-0428, after you sign the consent pages. Ask the office whether it went in.
- Prior authorization pending or deniedVera's checklist says plans commonly ask for the biopsy, a urine protein result and past medicines such as an ACE inhibitor or ARB. Vera's office checklist suggests a letter of medical necessity, because plans take time to set rules after an approval. For commercial insurance, the bridge program may supply a 28-day carton while coverage is pursued.
- A copay you cannot payCommercial insurance may qualify for the Tru Support copay program, as low as $0 up to a yearly maximum Vera does not publish. Medicare, Medicare Advantage, Medicaid and TRICARE are excluded by its terms. Uninsured or underinsured patients who meet an income limit may get it free through Vera's Patient Assistance Program. Our help paying page lists charity funds.

- A Patient Assistance Program that may give Trutakna at no cost to uninsured or underinsured patients who meet an income limit
- Nurse Educators who teach the injection and support you as you start, without giving medical advice
Questions prescribers' offices ask about Trutakna
What is the Trutakna dose?
150mg injected subcutaneously once weekly with a single-dose autoinjector, per the July 2026 label (DailyMed version dated August 5, 2026). There is no titration and no renal dose adjustment; a missed dose is given as soon as possible and weekly dosing resumes 1 week from it.
Are vaccines required before Trutakna?
The July 2026 label asks prescribers to complete all age-appropriate immunizations before the first dose, and says live vaccines are not recommended within 30 days before initiation or during treatment. No specific vaccine series is mandated, and Vera's prescriber site states no vaccines are required to start.
Is there a REMS for Trutakna?
No. The July 7, 2026 approval letter imposes no REMS and the label has no boxed warning; dispensing runs through select specialty pharmacies, which Vera's enrollment form lists as PANTHERx Rare Pharmacy and CVS Specialty Pharmacy.
What monitoring does the Trutakna label require?
None on a fixed schedule in the July 2026 label. It asks prescribers to assess for active infection before starting, delay initiation until an active infection resolves, monitor for infection during treatment, and consider interrupting Trutakna if a serious infection develops.
What do insurers ask for to approve Trutakna?
The Aetna Better Health Medicaid policy shown on this page, effective September 30, 2026, asks for biopsy-confirmed primary IgAN in an adult, proteinuria of 0.5g a day or more or UPCR of 0.5g/g or more within 3 months, at least 3 months of maximally tolerated ACEi or ARB, and a nephrologist. It approves 12 months at a time with a limit of 4 autoinjectors per 28 days.
