Prescribing companion
Vyvgart (efgartigimod alfa)And Vyvgart Hytrulo treatment cycles, administration, monitoring and prior authorization criteria for gMGFor prescribers and their offices treating generalized myasthenia gravis: what the pharmacy needs before it ships, what the prior authorization wants documented, the titration and the monitoring the label requires.
Dispensing gate
No REMS. Prior authorization, a site of care (infusion center, office, home nurse, or prefilled syringe self-injection after specialty pharmacy training), and no active infection on each dose day.
Example PA threshold
FEP example: age 18 or older, anti-AChR positive in a policy written before the May 2026 all-gMG label, MGFA II to IV, MG-ADL 5 or higher or QMG above 9, one failed AChE inhibitor or immunosuppressant, IgG 6g/L or higher.
Monitoring
Hypersensitivity observation for 1 hour after IV and at least 30 minutes after SC; infection check before each dose; vaccine review before each new cycle; no scheduled labs.

Prior authorization criteria and what to document
One public policy as the example: Blue Cross Blue Shield Federal Employee Program, January 1, 2026. Approval six months at first, then 12 months at a time if MG-ADL or QMG fell 2 points, with 49 days since the last cycle began. This policy was last reviewed in December 2025, before the FDA's May 2026 approval for all adults with gMG. It therefore still requires a positive AChR antibody test before approval. It asks for 1 hour of monitoring after every dose, although the Hytrulo label says at least 30 minutes. Its 49-day renewal rule is the plan's own, since the label has set no minimum gap between cycles since October 2025. IV Vyvgart is covered on FEP's medical benefit only, not its pharmacy benefit. It is here as an example of what a plan asks for, not as a rule.
| Element | What the policy asks | In the chart | Source |
|---|---|---|---|
| Diagnosis | Generalized myasthenia gravis, age 18 or older, with positive anti-AChR serology in this policy; the May 2026 label covers all adult gMG | Antibody result with date | FEP Blue policy |
| Severity | MGFA clinical class II to IV | Class recorded in the note | FEP Blue policy |
| Baseline score | MG-ADL total 5 or more, or QMG total above 9 | Dated score sheet | FEP Blue policy |
| Prior therapy | Inadequate response, intolerance or contraindication to one of: AChE inhibitor, azathioprine, cyclosporine, mycophenolate mofetil, tacrolimus, methotrexate, cyclophosphamide | Agent, dose, dates and outcome | FEP Blue policy |
| IgG | 6g/L or higher | Serum IgG with date | FEP Blue policy |
| Monitoring attestation | Monitoring during administration and for 1 hour after, for both products; the Hytrulo label says at least 30 minutes | Statement on the request | FEP Blue policy |
| Infection | No active infection, such as a UTI or respiratory infection | Note at the time of request | FEP Blue policy |
| Renewal | MG-ADL or QMG down 2 points or more from baseline, and at least 49 days since the start of the previous cycle; first approval 6 months, renewals 12 months | Current score against baseline, and cycle start dates | FEP Blue policy |
Vyvgart dosing and titration
| When | Label | Source |
|---|---|---|
| IV cycle | 10mg/kg IV over 1 hour once weekly for 4 weeks; 1,200mg per infusion at 120kg or more. Given by a healthcare professional. | FDA label, Vyvgart IV |
| SC cycle, prefilled syringe | 1,000mg efgartigimod alfa and 10,000 units hyaluronidase SC into the abdomen over about 20 to 30 seconds, once weekly for 4 weeks. Warm at least 30 minutes; rotate sites. | FDA label, Vyvgart Hytrulo |
| SC cycle, vial | 1,008mg and 11,200 units SC via a 25G winged infusion set over 30 to 90 seconds, once weekly for 4 weeks, by a healthcare professional only. Fixed dose; population PK found the effect of body weight not clinically relevant. | FDA label, Vyvgart Hytrulo |
| Next cycle | Administer subsequent cycles based on clinical evaluation. Study 1 protocol required 28 days after the last dose; the median time to cycle 2 was 50 days after the last dose of cycle 1. | FDA label, Vyvgart IV |
| Label history on cycle timing | The October 2025 supplement removed the statement that the safety of starting a cycle sooner than 50 days from the previous start was not established, and added safety data for cycles started 7 to 28 days after the last dose. | FDA letter |
| Missed dose | May be given up to 3 days after the scheduled time point; then resume the original schedule until the cycle is complete. | FDA label, Vyvgart Hytrulo |
Monitoring the Vyvgart label requires
| When | What | What the label says to do with it | Source |
|---|---|---|---|
| Each dose | Hypersensitivity: anaphylaxis, hypotension leading to syncope, angioedema, dyspnea, rash; urticaria with Hytrulo | Observe at least 30 minutes after SC (1 hour after IV per the Vyvgart label); trial reactions occurred within 1 hour to 3 weeks of administration | FDA label, Vyvgart Hytrulo |
| Each dose | Infusion or injection reactions: hypertension, chills, shivering, thoracic, abdominal and back pain | Severe: stop, treat and weigh readministration. Mild to moderate: rechallenge with close observation, slower rate and premedication | FDA label, Vyvgart IV |
| Before each dose | Active infection | Delay until resolved; if serious infection occurs, treat and consider withholding. UTI 10% against 5%, respiratory infection 33% against 29%; low WBC 12% against 5%, lymphocytes 28% against 19%, neutrophils 13% against 6% | FDA label, Vyvgart IV |
| Before each new cycle | Vaccination status | Evaluate age-appropriate vaccines per immunization guidelines; live vaccines not recommended during treatment | FDA label, Vyvgart IV |
| First SC cycle | Injection site reactions | 38% in Study 3, all mild to moderate, mostly within 24 hours, incidence falling with each cycle | FDA label, Vyvgart Hytrulo |
| Between cycles | MG-ADL and clinical status | Cycle timing rests on clinical evaluation; argenx states regular blood tests are not required | argenx FAQs |
Vyvgart drug interactions and contraindications
| Label category | Agents and what the label says | Source |

| CYP450 | Not metabolized by CYP450; CYP-mediated interactions unlikely | FDA label, Vyvgart IV |
| Live vaccines | Not recommended during treatment; for infants exposed in utero, weigh risks and benefits before live vaccines | FDA label, Vyvgart IV |
| Renal impairment | No adjustment in mild impairment; insufficient data in moderate or severe impairment | FDA label, Vyvgart IV |
| Pregnancy and lactation | No human data; no developmental harm in animal studies; pregnancy registry 1-855-272-6524; no lactation data | FDA label, Vyvgart IV |
| Contraindicated | Serious hypersensitivity to efgartigimod alfa products or excipients, and for Hytrulo to hyaluronidase | FDA label, Vyvgart Hytrulo |
Why a first fill stalls, and the fix
- Prior authorization pending or deniedThe FEP policy below asks for an antibody test, a symptom score and an earlier treatment that did not work. Policies written before May 2026, like that one, may still require a positive AChR antibody test. The neurologist's records usually answer what the plan is asking for.
- The prescription went to a pharmacy outside the networkargenx works with a set of specialty pharmacies for the prefilled syringe, including Accredo, CVS Specialty and Optum. A Nurse Case Manager can route the prescription to one in your plan's network, so this is a routing problem rather than a denial.
- The infusion site is not coveredThe Access Managers at My VYVGART Path verify insurance benefits and help locate a site of care. They also support the office through denials and appeals with your health plan.
- A copay you cannot payCommercial insurance: the VYVGART Co-pay Program, as little as $0 for the drug and its administration, up to $25,000 a calendar year. Medicare, Medicaid, Medigap, TRICARE, VA and uninsured patients are excluded by its terms. Our help paying page lists charity funds that may cover what remains.
- You live in Massachusetts or Rhode IslandThe Co-pay Program helps with the drug cost in these 2 states but not with administration costs. The infusion or injection fee from the clinic may therefore fall on you.
- Renewal refused after the first approvalThe FEP policy below approves 6 months first, then renews for 12 months if the MG-ADL or QMG score dropped 2 points or more. Keeping the score sheets from cycle 1 makes that renewal easier for the office.

Questions prescribers' offices ask about Vyvgart
What is the Vyvgart dose for generalized myasthenia gravis?
10mg/kg IV over 1 hour once weekly for 4 weeks, or 1,200mg (3 vials) per infusion at 120kg or more, per the May 2026 label. Vyvgart Hytrulo is a fixed 1,000mg/10,000 units by prefilled syringe or 1,008mg/11,200 units by vial, SC once weekly for 4 weeks. Later cycles follow clinical evaluation.
How soon can the next Vyvgart cycle start?
No minimum interval: the May 2026 labels say to give subsequent cycles based on clinical evaluation, after the October 2025 revision removed the statement that cycles sooner than 50 days from the previous start were not established. The label now cites 60 patients who started cycles 7 to 28 days after the last dose, with safety similar to Study 1. Some payers, including the FEP example here, still want 49 days from the previous cycle's start at renewal.
Can patients self-inject Vyvgart Hytrulo?
Yes, with the 1,000mg/10,000 units prefilled syringe, approved in April 2025 and carried in the May 2026 label, after proper instruction in SC technique. The vial is for administration by a healthcare professional only, with a winged infusion set. Monitor for hypersensitivity for at least 30 minutes after either.
Are vaccines or lab tests required before Vyvgart?
None of the 3 is a precondition in the May 2026 labels: there is no REMS, no required vaccine series and no scheduled lab. The labels ask prescribers to evaluate age-appropriate vaccines before each new cycle, say live vaccines are not recommended during treatment, and say to delay any dose during active infection.
What do insurers ask for to approve Vyvgart?
The FEP policy shown on this page, effective January 1, 2026 and written before the May 2026 label, asks for age 18 or older, positive anti-AChR antibodies, MGFA class II to IV, MG-ADL of 5 or more or QMG above 9, one failed conventional agent, IgG of 6g/L or more and no active infection. It approves 6 months, then 12 months on a 2-point MG-ADL or QMG improvement with at least 49 days since the previous cycle began.
