Your first 90 days
Your first 90 days on Vyvgart for generalized myasthenia gravis
What happens before the first fill, the days that matter, the label's rules and who to call, from the label, the drugmaker's own program and one insurer's written rules.
Vyvgart works in rounds of weekly doses rather than as a pill taken every day. Each cycle is 4 doses a week apart, followed by a break whose length the neurologist sets from how your symptoms behave. The first 90 days usually hold one full cycle and often the start of a second. This page lays out that stretch from the 2 FDA labels, argenx's My VYVGART Path program and one insurer's written rules.
What has to happen before the first bottle ships
Vyvgart has no REMS, so the first dose waits on an insurance approval, a place to be treated and, for self-injection, a training visit. These are the 6 things in the way, in order.
Where each dose will happen
The argenx What's Next Guide says your doctor works with you and your insurer to decide where treatment happens. Possible places include an infusion center, the doctor's office and, in some cases, your home with a trained nurse. It also says a specialty pharmacy or the office will call to go over coverage and costs. What's Next Guide
IV infusion or an injection under the skin
Vyvgart Hytrulo comes as a vial, which only a healthcare professional may give, or as a prefilled syringe. The label says patients or caregivers may use the prefilled syringe after proper instruction in injection technique. Vyvgart itself is an IV infusion that a healthcare professional gives over 1 hour.FDA label, Vyvgart Hytrulo
Training before the first self-injection
On argenx's prescriber enrollment form, the specialty pharmacy nurse gives 2 self-injection trainings, and more can be arranged on request. The form says needles, syringes and other supplies for home injection are provided as well. Separately, the Vyvgart Hytrulo Patient Information says not to inject until a healthcare provider has shown you how.Enrollment form
Vaccines, checked before each cycle
The label asks the doctor to check whether any age-appropriate vaccines are due before each new treatment cycle. Live vaccines are not recommended during treatment because Vyvgart lowers the body's IgG antibodies. Tell the neurologist about any shot you recently had or have scheduled.FDA label, Vyvgart IV
No active infection on dose day
The label says to delay a dose while an infection is active, until it has resolved. Tell the office about any fever, cough or urinary symptoms before you travel to the appointment or open a syringe.FDA label, Vyvgart IV
The insurance approval, and My VYVGART Path
Many plans require prior authorization, and argenx says each insurer's timeline varies. Once you are enrolled, a Nurse Case Manager calls to welcome you and a Welcome Kit with a symptom journal arrives by mail. For commercial insurance with access delays, the Quick Start Program may supply Vyvgart Hytrulo for a limited time at no cost.My VYVGART Path
The days that matter
Counted from the first dose, with the 4 weekly doses of cycle 1, the vaccine check, and where argenx and the trial put cycle 2. Type your first-dose date and the tiles become real dates you can add to your calendar.
The tiles below count days from a first dose. Enter the date of yours, or the date you expect, and each tile becomes a real date on your calendar.
The date stays on your device. It is not sent to Trial Friend or anyone else. The calendar file works with Apple, Google and Outlook calendars.Enrollment and the benefits check
The neurologist's office sends the My VYVGART Path enrollment form, and the team checks your benefits and prior authorization rules. No source sets how long this takes, since each insurer's timeline differs.
Approval, then the scheduling call
Once the prior authorization is approved, a specialty pharmacy or the doctor's office calls about coverage, location and cost. For the prefilled syringe, the pharmacy books at least 2 training sessions and ships the syringes to your home.
First dose of cycle 1
One dose this week, then 1 a week for 3 more weeks. The prefilled syringe takes about 20 to 30 seconds, with at least 30 minutes of watching for an allergic reaction afterward. A carton of 4 prefilled syringes holds the whole of one cycle.
Dose 2
Same dose, given on the same day of the week as the first one. If it is missed, the label allows it up to 3 days late, and then the original schedule resumes until the cycle ends.
Dose 3
Injection site redness, bruising or itching was most common in the first cycle and became less frequent with each later cycle, according to the label. In the Hytrulo trial, all of these reactions were mild to moderate.
Dose 4, the last of cycle 1
This dose ends cycle 1, and the label sets no fixed break before the next one. The neurologist times cycle 2 from a clinical evaluation of how you are doing.
The break, and symptom tracking
argenx's dosing page says to contact your doctor if symptoms change during the break, without waiting for a visit. It suggests tracking them on the MG-ADL scale, which scores 8 daily activities such as chewing and breathing.
Vaccine check before the next cycle
The label asks the doctor to check due vaccines before every new cycle, not only the first one. Mention any shot you have had or planned since the last cycle.
Cycle 2 in argenx's example
argenx's dosing page gives a 4-week break after each of the first 3 cycles as one example. Counted from day 22, that puts the first dose of cycle 2 here, with a new carton or infusion booked beforehand.
When trial patients started cycle 2
In the main trial, Study 1 in the label, the median wait for a second cycle was 50 days after the last dose. Trial rules required a wait of at least 28 days, and your neurologist may choose a shorter or longer break.
The label, revision by revision
The FDA lists which sections changed in each revision of a label. These are the revisions the current Vyvgart label records, dated by the FDA, with a plain note on what each one meant for a patient. Older references and handouts can lag these by months.
Current label dated July 15, 2026, read from the FDA's label database today.
- May 2026
The approved use widened from adults with AChR antibodies to all adults with generalized myasthenia gravis, for both Vyvgart and Vyvgart Hytrulo. The label added a trial in 119 adults without AChR antibodies. The dose and the rules for treatment cycles did not change with this approval.FDA letter
- February 2026
The FDA required new safety wording on the risk of severe worsening of CIDP symptoms after starting Vyvgart Hytrulo. The change sits in the CIDP dosing section and did not touch myasthenia gravis dosing.FDA letter
- October 2025
The FDA removed the statement that cycles started sooner than 50 days after the previous start had not been shown safe. The label added a study of 60 patients who began cycles 7 to 28 days after their last dose.FDA letter
- April 2025
The Vyvgart Hytrulo prefilled syringe was approved, and the label began allowing patients or caregivers to give it at home after training.FDA letter
- August 2024
The FDA approved new vaccination wording in the dose and warnings sections of both labels.FDA letter
- July 2024
Details of the pregnancy exposure registry were added to the Vyvgart label.FDA letter
How the label says to take it
The parts of the prescribing information and the patient leaflet that come up in the first 3 months, in plain words.
- One dose a week for 4 weeks, then a break
The label calls this run of 4 weekly doses a single treatment cycle. Later cycles are given according to clinical evaluation, which means the neurologist decides from your symptoms and side effects rather than from a fixed calendar.FDA label, Vyvgart Hytrulo
- How the IV dose is set
Vyvgart is 10mg/kg by body weight, given over 1 hour through a filter. People weighing 120kg (about 265 lb) or more get 1,200mg, which is 3 vials. Staff watch you during the infusion and for 1 hour afterward.FDA label, Vyvgart IV
- The prefilled syringe at home
The Instructions for Use say to let the syringe warm out of the fridge for at least 30 minutes, never with a heat source. The label places the injection in the belly at least 2 inches from the navel, with a new spot each time. It also calls for watching for allergic signs for at least 30 minutes afterward.FDA label, Vyvgart Hytrulo
- If a dose is missed
The label allows a missed dose to be given up to 3 days after the scheduled day. After that, the original schedule resumes until the cycle is complete.FDA label, Vyvgart Hytrulo
- Storage
The label keeps syringes in the fridge at 36°F to 46°F (2°C to 8°C) in the original carton, never frozen or shaken. If needed, one can sit at room temperature up to 86°F (30°C) for a single stretch of up to 30 days. The Patient Information says not to put it back in the fridge once it has warmed.FDA label, Vyvgart Hytrulo
- Other antibody medicines
Vyvgart can lower the levels and weaken the effect of drugs that use the same FcRn receptor, such as immunoglobulin (IVIG) and some monoclonal antibodies. The label asks doctors to watch for this and, if long-term use of both is essential, to consider stopping Vyvgart.FDA label, Vyvgart IV
- Your other myasthenia medicines
argenx says patients in the studies stayed on their usual oral myasthenia medicines during treatment. Any change to those medicines is a decision for your neurologist.argenx FAQs
- Pregnancy and breastfeeding
There are no human data on Vyvgart in pregnancy, and animal studies showed no harm to development. Because it lowers the mother's IgG, a baby exposed in the womb may have less protection. Live vaccines for that baby need a risk review, and it is not known whether Vyvgart passes into breast milk. A pregnancy registry for Vyvgart takes enrollments by phone at 1-855-272-6524.FDA label, Vyvgart IV
What the label says to act on
The symptoms the label singles out, and what it tells patients and prescribers to do about each. If any of these happen, the label's instruction is the one to follow, with your care team.
Signs of an allergic reaction
Rash, hives, swelling of the face, lips, throat or tongue, shortness of breath, trouble breathing, low blood pressure or fainting. The Patient Information says to tell your provider or get emergency help right away. The label says trial reactions came anywhere from within an hour to 3 weeks after a dose.FDA label, Vyvgart Hytrulo
Signs of infection
Fever, chills, frequent or painful urination, cough, a blocked nose, wheezing, shortness of breath, sore throat, phlegm or a runny nose. The Patient Information says to tell your provider right away. In the main trial, respiratory infections occurred in 33% on Vyvgart against 29% on placebo, and urinary infections in 10% against 5%.FDA label, Vyvgart Hytrulo
Infusion or injection reactions
High blood pressure, chills, shivering, or chest, stomach or back pain during or soon after a dose. In the Hytrulo trial, 38% had injection site reactions, all mild to moderate, mostly within a day and fewer with each later cycle. Headache occurred in 32% on Vyvgart against 29% on placebo in the main trial.FDA label, Vyvgart Hytrulo
Why the pharmacy or the insurer says no, and what fixes it
Most first-fill delays are one of these. Knowing which one tells you who to call.
- Prior authorization pending or deniedThe FEP policy below asks for an antibody test, a symptom score and an earlier treatment that did not work. Policies written before May 2026, like that one, may still require a positive AChR antibody test. The neurologist's records usually answer what the plan is asking for.FEP Blue policy
- The prescription went to a pharmacy outside the networkargenx works with a set of specialty pharmacies for the prefilled syringe, including Accredo, CVS Specialty and Optum. A Nurse Case Manager can route the prescription to one in your plan's network, so this is a routing problem rather than a denial.Prefilled syringe FAQs
- The infusion site is not coveredThe Access Managers at My VYVGART Path verify insurance benefits and help locate a site of care. They also support the office through denials and appeals with your health plan.My VYVGART Path overview
- A copay you cannot payCommercial insurance: the VYVGART Co-pay Program, as little as $0 for the drug and its administration, up to $25,000 a calendar year. Medicare, Medicaid, Medigap, TRICARE, VA and uninsured patients are excluded by its terms. Our help paying page lists charity funds that may cover what remains.Co-pay Program terms
- You live in Massachusetts or Rhode IslandThe Co-pay Program helps with the drug cost in these 2 states but not with administration costs. The infusion or injection fee from the clinic may therefore fall on you.Co-pay Program terms
- Renewal refused after the first approvalThe FEP policy below approves 6 months first, then renews for 12 months if the MG-ADL or QMG score dropped 2 points or more. Keeping the score sheets from cycle 1 makes that renewal easier for the office.FEP Blue policy
Blue Cross Blue Shield Federal Employee Program prior authorization policy, January 1, 2026
- Age 18 or older, with generalized myasthenia gravis and a positive AChR antibody test
- Myasthenia Gravis Foundation of America class II to IV
- A baseline MG-ADL score of 5 or more, or a QMG score above 9
- An acetylcholinesterase inhibitor, azathioprine, cyclosporine, mycophenolate mofetil, tacrolimus, methotrexate or cyclophosphamide did not work well enough, was not tolerated or could not be used (one of these)
- An IgG level of 6g/L or higher
- The prescriber agrees to monitor during each dose and for 1 hour after for allergic reactions
- No active infection, such as a urinary or respiratory infection
Approval length. Six months at first, then 12 months at a time if MG-ADL or QMG fell 2 points, with 49 days since the last cycle began.
This policy was last reviewed in December 2025, before the FDA's May 2026 approval for all adults with gMG. It therefore still requires a positive AChR antibody test before approval. It asks for 1 hour of monitoring after every dose, although the Hytrulo label says at least 30 minutes. Its 49-day renewal rule is the plan's own, since the label has set no minimum gap between cycles since October 2025. IV Vyvgart is covered on FEP's medical benefit only, not its pharmacy benefit. It is here as an example of what a plan asks for, not as a rule. FEP Blue policy
Who to call, and what they can do

Five questions for the pharmacist or the support line
- Which specialty pharmacy or infusion center is handling my doses, and what number will they call me from?
- When are my self-injection training sessions, and can my caregiver be trained at the same time?
- Does one carton of 4 syringes cover my whole cycle, and when should the next carton be ordered?
- If my dose day falls on a holiday or I am traveling, how many days late can it be given?
- I take IVIG or another antibody medicine. Does my neurologist need to know before my next cycle?
- Has my co-pay card been applied, and does it cover the clinic's administration fee as well as the drug?
Questions people ask in the first weeks
How long does it take to start Vyvgart after it is prescribed?
There is no fixed number of days between the prescription and the first dose. Benefits verification, prior authorization, choosing a site of care and, for self-injection, training each take their own time. argenx says each insurer's timeline varies, and its Quick Start Program exists for commercially insured patients facing delays.
How often is Vyvgart given for myasthenia gravis?
In cycles of 1 dose a week for 4 weeks. The label leaves the timing of later cycles to the doctor's clinical evaluation and no longer sets a minimum break. In the main trial the median gap was 50 days after the last dose of the first cycle.
Can I give Vyvgart Hytrulo to myself at home?
Yes, with the prefilled syringe, after a healthcare provider has trained you or a caregiver. The FDA approved the Vyvgart Hytrulo prefilled syringe for home use in April 2025. The vial form of Vyvgart Hytrulo is given only by a healthcare professional.
Do I need vaccines before starting Vyvgart?
No specific vaccine is required to start, according to argenx. The label asks the doctor to check age-appropriate vaccines before each new cycle and says live vaccines are not recommended during treatment.
Do I need blood tests on Vyvgart?
The label sets no scheduled blood test, and argenx says regular blood tests are not required. The neurologist still follows your symptoms and side effects to decide when each cycle starts.
What if I have an infection when a dose is due?
The label says to delay the dose until the infection has resolved. Call the neurologist's office before the appointment rather than after it, so the visit or delivery can be moved.
Is Vyvgart approved if I do not have AChR antibodies?
Yes, since the FDA expanded both forms in May 2026 to all adults with generalized myasthenia gravis. argenx says that covers MuSK positive, LRP4 positive and triple seronegative disease. Some insurer policies written earlier still ask for a positive AChR test.
Is Vyvgart Hytrulo used the same way for CIDP?
No, for chronic inflammatory demyelinating polyneuropathy the label gives Vyvgart Hytrulo once a week without cycles or breaks. This page covers only generalized myasthenia gravis, which is the use with treatment cycles.
