A refill of eltrombopag, the daily pill many people with immune thrombocytopenia (ITP) take to keep their platelets up, can now arrive in 3 forms: Novartis's brand Promacta, an AB-rated generic from Camber, Zydus or another maker, or Teva's Alvaiz. ITP is the condition in which the immune system destroys platelets, the blood cells that stop bleeding, and the version in the bottle matters in 2 ways. A generic carries the same milligram dose as Promacta, and a pharmacist can usually swap it in. Alvaiz uses different numbers and needs its own prescription. All 3 share the same calcium rule, which decides how much of each dose reaches the blood.
Promacta had no US generic until May 14, 2025, when Camber launched the first AB-rated version, according to Camber and an Optum Rx notice to pharmacists. Zydus filed its own list prices with Vermont's attorney general on January 14, 2026, and the FDA has approved generic eltrombopag from several companies. Alvaiz came first, approved on November 29, 2023, through an FDA pathway for new versions of approved drugs rather than as a generic, so it is not interchangeable at the pharmacy counter. Novartis still sells Promacta.
Is Generic Eltrombopag the Same as Promacta for ITP?
Every AB-rated generic eltrombopag is the same drug as Promacta at the same dose. The FDA rates the generics AB against Promacta, meaning they contain the same active ingredient, eltrombopag olamine, in the same strengths, and a pharmacist can substitute one for the other where state law allows. Camber sells 12.5, 25, 50 and 75 mg tablets, the same strengths as Promacta, plus oral suspension packets for young children.
Everything on the Promacta label carries over. For ITP in adults and children 6 and older, it starts at 50 mg once a day. Children aged 1 to 5, people of East or Southeast Asian ancestry and people with liver impairment start at 25 mg, and 12.5 mg can be considered for someone with both Asian ancestry and liver impairment. Doctors raise the dose in 25 mg steps, no higher than 75 mg a day, aiming for a platelet count of at least 50,000, enough to lower bleeding risk rather than a normal count. The label calls for liver blood tests before starting, every 2 weeks while the dose is being adjusted and monthly after that, plus weekly blood counts until platelets are stable. Promacta and its generics carry a boxed warning, the FDA's strongest, for liver injury.
Switching between Promacta's tablet and its liquid suspension is a change the label singles out, with weekly platelet checks for 2 weeks afterward. Families of young children who move from packets to tablets should expect that schedule whether the product is brand or generic.
Why Does Alvaiz Start at 36 mg When Promacta Starts at 50 mg?
Alvaiz contains eltrombopag choline, a different salt form of the same medicine, which the body absorbs differently from the eltrombopag olamine in Promacta, so the Alvaiz label sets its own milligram doses: 36 mg where Promacta starts at 50 mg. Neither number is the stronger treatment. The Alvaiz label says it is not substitutable with other eltrombopag products on a milligram per milligram basis, and the FDA approved it using Promacta's clinical trials plus studies of how Alvaiz is absorbed.
Lined up side by side, the 2 labels follow a single ratio. Every Alvaiz dose is 72% of the matching Promacta dose: 36 of 50 mg to start, 18 of 25 mg for Asian ancestry or liver impairment, 9 of 12.5 mg when both apply, 18 of 25 mg for each step up, and 54 of 75 mg at the ceiling. Trial Friend worked that out from the dosing sections of both FDA labels on October 11, 2026; neither label states it, and only a prescriber should set a dose when someone moves between the 2.
| Promacta and generic eltrombopag | Alvaiz | |
|---|---|---|
| Active ingredient | Eltrombopag olamine | Eltrombopag choline |
| Usual starting dose | 50 mg once a day | 36 mg once a day |
| Reduced start (Asian ancestry or liver impairment) | 25 mg | 18 mg |
| Highest daily dose | 75 mg | 54 mg |
| Youngest age for ITP | 1 year | 6 years |
| Swapped at the pharmacy | Yes, AB-rated generics replace Promacta where state law allows | No, Alvaiz needs its own prescription |
| Drugmaker copay help | Novartis for Promacta, Camber for its generic; commercial insurance only | Teva; commercial insurance only |
Does the Dairy and Calcium Rule Apply to Generic Eltrombopag and Alvaiz?
The calcium rule applies to every eltrombopag product. Promacta, its generics and Alvaiz are all taken without a meal or with a meal containing 50 mg of calcium or less, and at least 2 hours before or 4 hours after antacids, calcium-rich foods such as milk, yogurt and cheese, and supplements with iron, calcium, magnesium, aluminum, selenium or zinc. Calcium and those other minerals bind eltrombopag in the gut and keep it from being absorbed.
The Promacta label measures what breaking that rule costs. A high-fat breakfast that included dairy and carried 427 mg of calcium cut the amount of eltrombopag reaching the blood from a 50 mg tablet by about 59%, and an antacid cut it by about 70%. A separate study of the liquid suspension in healthy adults timed a 25 mg dose around a meal with 448 mg of calcium. Taken with the meal, 75% of the usual exposure was lost, leaving about 25%. Taken 2 hours after the meal, about 53% got through. Taken 2 hours before it, about 80% did.
Taking eltrombopag with a meal containing 448 mg of calcium left about 25% of the usual amount in the blood; taking it 2 hours after the meal left about 53%, and 2 hours before left about 80%. Trial Friend converted the label's reported reductions (75%, 47% and 20%) into the share that got through. The study used the oral suspension in healthy adults, and the Promacta and Alvaiz labels apply the same timing rule to every form.
Even a dose taken at the edge of the allowed window, 2 hours before the meal, lost about a fifth of its exposure in that study. The rule itself did not change when generics and Alvaiz arrived. What changed is that someone moving between products may also change the time of day they take the pill, and the care team should hear about that at the next platelet check.
What Promacta, Generic Eltrombopag and Alvaiz Cost on Commercial Insurance, Medicare and Medicaid
People with commercial insurance can apply for drugmaker help with any of the 3, and people on Medicare or Medicaid cannot use those cards at all. Novartis's Universal Co-pay Program lets eligible privately insured patients pay $0 a month for Promacta, with Novartis covering up to $15,000 a year. Camber's launch announcement said eligible patients may pay as little as $0 through its copay program for the generic. Teva's Alvaiz Savings Program offers as little as $0 out of pocket to people whose commercial plan covers Alvaiz, and its current card expires December 31, 2026. Each program excludes Medicare, Medicaid and other government coverage (program terms checked between September 24 and October 11, 2026).
List prices show why the Medicare cap matters. Zydus reported to Vermont a wholesale acquisition cost, the price a drugmaker charges wholesalers before discounts, of $3,349.52 for 30 tablets of 50 mg generic eltrombopag, and $1,850.89 for 30 tablets of either 12.5 mg or 25 mg. That single 50 mg bottle lists for more than the whole 2026 Medicare Part D yearly cap on out-of-pocket drug costs, $2,100, which rises to $2,400 in 2027 (CMS). What a pharmacy or plan actually pays is often below list price, and your own cost depends on your plan.
Medicare open enrollment runs from October 15th to December 7th. Check whether your 2027 plan lists Promacta, generic eltrombopag, Alvaiz or all 3, and what prior authorization each needs, since a plan can prefer one version over another. Our Medicare open enrollment guide walks through those checks. Charity funds that help Medicare patients with ITP drug costs open and close often. When Trial Friend checked them on October 5, 2026, HealthWell's and TotalAssist's ITP funds were closed and The Assistance Fund's thrombocytopenia fund had a waitlist. The ITP help paying section and the patient assistance directory keep the current status.

Why Some Insurers Want Eltrombopag Tried Before Wayrilz
Some insurers will not pay for Wayrilz (rilzabrutinib), the twice-daily ITP pill from Sanofi approved on August 29, 2025, until a patient has tried other treatments first, and eltrombopag is on those lists. This is step therapy, an insurer rule that a cheaper or older drug must fail before a newer one is covered. Two published policies show how differently it can work.
Centene's rule on combinations is tighter than how Wayrilz was studied. In LUNA 3, the trial behind its approval, 60% of people on Wayrilz also took steroids, a TPO receptor agonist or both at stable doses, according to the Wayrilz label. Under the Centene policy, someone whose platelets respond to Wayrilz only alongside eltrombopag may not get both covered. In that trial, 23% of people on Wayrilz (31 of 133) had a durable platelet response, meaning counts of at least 50,000 at two-thirds or more of weekly checks over the last 12 weeks of treatment without rescue medicine, against none of 69 on placebo.
A denial can be appealed, and a doctor can ask for an exception when an earlier drug caused side effects or is medically unsuitable. Our guide to appealing an insurance denial for a rare disease drug covers the letters and deadlines. Sanofi's HemAssist program offers copay help for commercially insured patients and a free first month of Wayrilz for a first prescription, except in Vermont.
Doptelet, Nplate, Tavalisse and Wayrilz When Eltrombopag Is Not Enough
Four other FDA-approved treatments are used for ITP that has not responded well enough to a first treatment, and their daily routines differ. Doptelet is the only one of them taken with food, and Nplate is the only injection. Our ITP treatment comparison sets out how each was tested, the side effects and what to ask before choosing.
| How often | Food and timing | |
|---|---|---|
| Eltrombopag (Promacta, generics, Alvaiz), from age 1, Alvaiz from 6 | Pill once a day | No meal or a low-calcium meal; 2 hours before or 4 hours after dairy, antacids and minerals |
| Doptelet (avatrombopag), from age 1 | Pill once a day | Take with food; no calcium rule |
| Nplate (romiplostim), from age 1 | Injection once a week by a healthcare provider | No food rules |
| Tavalisse (fostamatinib), adults | Pill twice a day | With or without food |
| Wayrilz (rilzabrutinib), adults | Pill twice a day | With or without food; avoid proton pump inhibitors; at least 2 hours before an antacid or H2 blocker |
Someone who struggles to fit eltrombopag's calcium window around meals can ask a hematologist about Doptelet, made by Sobi, or Nplate, made by Amgen, neither of which has a calcium rule. Tavalisse, from Rigel, works on the immune side instead, as does Wayrilz. Their trials enrolled different patients and measured response in different ways, so the choice rests on each person's response, side effects and coverage rather than a ranking.
ITP Clinical Trials That Add to Eltrombopag, and the Ianalumab Filing Planned for 2027
The next ITP treatment may be designed to work alongside eltrombopag rather than replace it. In Novartis's phase 3 VAYHIT2 trial, adults who took 4 monthly infusions of ianalumab, an antibody that depletes B cells, the immune cells that make the antibodies attacking platelets in ITP, together with eltrombopag went a median of 13.0 months before their treatment failed, against 4.7 months for eltrombopag with a placebo, Novartis reported on December 9, 2025. Novartis says it plans to file with regulators in 2027 once results from VAYHIT1, a trial in newly diagnosed ITP, are in. Until a filing is accepted, any approval date is an estimate.
Novartis is also recruiting adults with ITP for a phase 2 study that pairs ianalumab with whichever TPO receptor agonist their doctor chooses, NCT07421167, and all 16 of its sites listed on ClinicalTrials.gov on October 11, 2026 were in the US. Other recruiting adult studies include argenx's phase 3 trial of efgartigimod infusions and Takeda's phase 3 trials of mezagitamab. The ITP trial finder lists every open study with its locations and updates daily.
Generic Promacta and Alvaiz, Short Answers
Is generic Promacta as good as brand Promacta?
The FDA rates generic eltrombopag AB against Promacta, meaning it is judged to work the same at the same dose. Its tablets may look different, and families switching a child from Promacta's oral suspension to tablets should expect weekly platelet checks for 2 weeks, as the label advises for that change.
Can I switch from Promacta to Alvaiz?
Yes, with a new prescription, because Alvaiz is not substituted at the pharmacy. The Alvaiz label sets its own doses, starting at 36 mg for most people and topping out at 54 mg a day, and Alvaiz is approved for ITP only from age 6, while Promacta is approved from age 1.
Can I take eltrombopag with milk?
No. Promacta, its generics and Alvaiz should be taken at least 2 hours before or 4 hours after milk, yogurt, cheese, calcium-fortified foods, antacids and mineral supplements. A meal is fine only if it contains 50 mg of calcium or less.
Does the Promacta copay card work for generic eltrombopag?
No. Novartis's Universal Co-pay Program covers brand Promacta. Camber runs its own copay program for its generic, and Teva runs the Alvaiz Savings Program, and all 3 programs are limited to people with commercial insurance.
Do I need to try generic Promacta before Wayrilz?
It depends on the insurer. Centene's plans require a steroid and generic Promacta to have failed or be unsuitable before covering Wayrilz, while UnitedHealthcare accepts any 2 of eltrombopag, Doptelet and Tavalisse. A doctor can request an exception when an earlier drug caused side effects or cannot be used.
When could ianalumab be approved for ITP?
Novartis plans to file in 2027 after its VAYHIT1 trial in newly diagnosed ITP reads out. For a new medicine, the FDA's standard review goal is 10 months after it accepts a filing, a step that itself takes up to 60 days, or 6 months under priority review, so no approval date can be set until Novartis files.
