Sam was born in January 1995, the same month a team in France reported the gene behind spinal muscular atrophy in the journal Cell. Sam is not a real person, and Sam's story is made up of experiences common to adults with SMA type 3. For most of Sam's childhood there was no medicine for SMA at all, and the first one arrived when Sam was 21.
At 31, Sam still walks and is weighing a question that did not exist 2 years ago. The question is whether to try a gene therapy that is given only once. That therapy is Itvisma, made by Novartis and injected a single time into the spinal fluid. The FDA approved it on November 24, 2025 for people with SMA aged 2 and older, adults included (FDA, 2025). This guide follows Sam's question through the evidence, the insurance rules and the trials enrolling adults now.
What Itvisma Is, and How It Differs From Zolgensma for Older Children and Adults
Itvisma (onasemnogene abeparvovec-brve) is a one-time gene therapy for people with SMA aged 2 and older. A single injection into the spinal fluid delivers a working copy of the SMN1 gene. Its label calls for a fixed dose of 1.2 × 10¹⁴ vector genomes for every patient, whatever their weight (Itvisma label, 2026).
Zolgensma carries the same gene in the same AAV9 shell, but it goes into a vein at a dose set by body weight. The FDA approved Zolgensma in May 2019 for children under 2. Itvisma's fixed dose and its route through a lumbar puncture, the same route Spinraza uses, are what make it practical for older and heavier patients.
The label carries a boxed warning, the FDA's strongest kind, for serious liver injury. People take a steroid called prednisolone from the day before the injection, at full dose for 30 days and then tapered over about 4 more weeks. Liver tests continue for at least 3 months. Other warnings on the label include low platelet counts and a blood clotting disorder called thrombotic microangiopathy. The label also lists nerve changes affecting feeling and raised levels of troponin, a heart protein.
Spinal Muscular Atrophy Science in One Lifetime, From the SMN1 Gene to Itvisma
Every treatment Sam can choose from today arrived within Sam's own lifetime, and several began with parents rather than companies. The timeline below places each milestone against Sam's age, from a parents' group founded 11 years before Sam was born to adult studies expected to report in the 2030s.
- 1984before birth
- Jan 1995age 0A team in France reports in Cell that changes in the SMN1 gene cause SMA (Lefebvre et al., 1995).
- 2003age 8Loren Eng and Dinakar Singh, parents of a child with SMA, start the SMA Foundation to fund research.
- Dec 2016age 21The FDA approves Spinraza, the first SMA treatment, given into the spinal fluid every 4 months after loading doses.
- Jul 2018age 23SMA joins the national newborn screening list after a nomination led by Cure SMA and partners.
- Aug 2026age 31Novartis starts STREAM, a US study of Itvisma in everyday care that enrolls adults (NCT07448610).
- Sep 2026age 31
- Oct 2026age 31Today, Sam is 31.
- Jun 2028estimateMain results are expected from SOLAR, Biogen's trial of a once-a-year spinal injection for people aged 15-60 (NCT07444476).
- May 2032estimateMain results are expected from STREAM, though estimated dates like this often move as studies enroll.
Sam, an example adult with SMA born in January 1995, was 21 when the first SMA treatment was approved and 30 when Itvisma was approved for adults. Trial Friend checked every date on October 24, 2026 against FDA approval records, the 1995 Cell paper and ClinicalTrials.gov. HRSA, Cure SMA, the SMA Foundation and Roche supplied the remaining milestones.
Two family-run groups appear on that line before any company does. Families of SMA began in 1984 as a small circle of parents. As Cure SMA, it later helped lead the nomination that put SMA on the national newborn screening list in 2018 (Cure SMA, 2018). By 2024 every state and Washington, DC screened newborns for SMA. Many babies now start treatment before symptoms begin, something no one could offer Sam's parents.
Loren Eng and Dinakar Singh started the SMA Foundation in 2003 as parents of a child with SMA. The foundation formed a research partnership with PTC Therapeutics that Roche joined in 2011 (Roche). That partnership produced risdiplam, the medicine sold today as Evrysdi.
Who Was in the Itvisma Trials, and What That Means for Adults With SMA
Both main Itvisma trials enrolled only people aged 2-17, according to their ClinicalTrials.gov records and published results. Adults are on the label because the FDA accepted the company's case for extending the trial findings to them, a step the agency described in its approval announcement (FDA, 2025). The same announcement said the label's warnings reflect possibly higher risks for adults living with other chronic conditions. That is one more reason the adult conversation belongs with a neurologist who knows your health history.
STEER, the larger trial, enrolled children and teens aged 2-17 who could sit but had never walked on their own and had not been treated before. The trial measured motor skills with the HFMSE, a standard test. Averaged across weeks 48 and 52, scores rose 2.39 points with Itvisma against 0.51 points with a sham procedure. That difference of 1.88 points was the trial's main result (Proud et al., Nature Medicine, 2026). STRENGTH followed 27 people aged 2-17 who switched from Spinraza or Evrysdi. It reported no deaths and no one leaving the trial because of side effects (Kwon et al., Nature Medicine, 2026).
By Trial Friend's count from those 2 records, 102 people were followed for a full year after Itvisma, 75 in STEER and 27 in STRENGTH. Everyone was aged 2-17 at enrollment, and STEER's sham group was offered Itvisma after week 52.
That gap in the evidence for adults is starting to close. STREAM is a Novartis study that began on August 27, 2026 and follows people who receive Itvisma in everyday US care. Its enrollment groups include adults who walk on their own and adults who do not (NCT07448610).
One Injection or About 33, What 10 Years on Each SMA Treatment Looks Like
Over 10 years, standard Spinraza adds up to about 33 spinal injections and Evrysdi to about 3,652 daily doses, while Itvisma is a single injection. Trial Friend counted these from each FDA label's dosing schedule. That count is not a measure of how well any of them works, since no trial has compared Itvisma with Spinraza or Evrysdi directly.
Over 10 years, Spinraza adds up to about 33 spinal injections on the standard schedule and about 31 on the higher-dose schedule. Evrysdi is about 3,652 daily doses, and Itvisma is 1 injection. Itvisma also brings about 2 months of prednisolone and at least 3 months of liver checks, and its label says people who already had Zolgensma should not receive it.
The numbers mean different things to different people. A spinal injection every 4 months is a real burden for some adults, especially those who find lumbar punctures difficult. Others find a daily dose at home easy to fit into life. Itvisma trades those routines for a single procedure, a steroid course and close monitoring in the months after.
What 5 Insurers Require Before Covering Itvisma, Side by Side
Trial Friend read 5 published insurer policies on October 24, 2026. Aetna's and Cigna's policies cover Itvisma only for ages 2-17. UnitedHealthcare, Anthem and the Blue Cross Blue Shield Federal Employee Program set no upper age, which matches the FDA label.
- FDA labelApproved November 24, 2025
- UnitedHealthcare2 or 3 SMN2 copies; no routine use alongside Spinraza or Evrysdi
- Anthem (Elevance)3 or fewer SMN2 copies; no planned Spinraza or Evrysdi afterward
- BCBS Federal Employee Program3 or fewer SMN2 copies; no use at the same time
- Aetna3 or fewer SMN2 copies; stop Spinraza or Evrysdi before the dose
- Cigna2 or 3 SMN2 copies, or 4 with a test that separates 4 from 5; stop Spinraza or Evrysdi first
Aetna's and Cigna's published policies cover Itvisma only for ages 2-17. UnitedHealthcare, Anthem and the Blue Cross Blue Shield Federal Employee Program set no upper age, matching the FDA label. Humana's criteria were not public, Medicare has no national coverage decision, and employer plans can set their own rules. Trial Friend read UnitedHealthcare policy 2026D0142C, Aetna bulletin 0953, Cigna policy IP0790, Anthem policy MED.00129 and FEP policy 5.75.047 on October 24, 2026.
None of the 5 policies sets a waiting period between the last Spinraza or Evrysdi dose and Itvisma. STRENGTH required at least 4 months after Spinraza and 15 days after Evrysdi, so expect your neurologist to plan around those gaps even when the policy is silent.
Lab rules differ from one insurer to the next as well. Cigna's policy asks for liver tests no higher than twice the normal limit within 30 days before treatment, along with kidney and blood count checks. UnitedHealthcare's policy centers on the antibody test and the steroid plan. For help with the paperwork, Novartis Patient Support at 1-855-441-4363 assigns a case coordinator and runs a copay program for people with private insurance. Trial Friend keeps the current Itvisma program details on its help paying page.

Weighing Itvisma for SMA at Different Ages, 4 Situations and the Question Each One Brings
These 4 people are examples built from common situations, not real patients. Each one ends with a question to bring to the neurologist.
A 16-year-old who has had Spinraza since age 8
This teenager is the closest match to the STRENGTH trial, which studied people aged 2-17 who switched from Spinraza or Evrysdi. The question is how long Spinraza would pause before Itvisma, and what the plan says about stopping it for good.
A 34-year-old who walks and has never been treated
No trial has given Itvisma to someone like this yet, so the decision rests on the label, the neurologist's judgment and the insurer. The questions are whether the plan follows the label's open age range and whether STREAM, which enrolls adults who walk on their own, is an option.
A 17-year-old on an Aetna or Cigna plan
Both policies end at age 17, so timing may matter for this family in a way it does not for others. The question to ask, calmly and early, is how the plan handles a request made before the 18th birthday.
A parent of a 4-year-old with 3 SMN2 copies
This child sits inside the ages studied in STEER and inside every insurer's SMN2 rule in the coverage chart above. The question is how Itvisma compares with the child's current treatment over the next 10 years, including the steroid course and the liver checks.
Cure SMA and the SMA Foundation, the Families Behind Today's SMA Treatments
Cure SMA runs a teens and adults resource section and offers an Adults with SMA Scholarship for its annual conference. The scholarship covers registration, a travel stipend and 1 hotel room for 3 nights, shared by the adult with SMA and a personal care assistant. The next Annual SMA Conference runs June 17-20, 2027 in Anaheim, California, with social events for teens and adults.
Unlike Cure SMA, the SMA Foundation focuses on research rather than family services. It says it has spent about $150 million on SMA research since 2003 (SMA Foundation). Families who want to follow the science can read its updates, while adults looking for others with SMA often start with Cure SMA's programs.
What Comes Next for Adults With SMA in 2027, and the Trials Enrolling Now
Five studies enrolling at US sites accept adults with SMA, by Trial Friend's search of ClinicalTrials.gov on October 24, 2026. They range from a once-a-year injection to a port that replaces repeated spinal taps.
- STREAM (NCT07448610) from Novartis follows people who receive Itvisma in routine US care. Its groups cover children and teens who walk, adults who walk and adults who do not. The study began on August 27, 2026, plans about 36 participants and lists 1 US site in New York.
- SOLAR (NCT07444476) from Biogen tests salanersen, an experimental medicine designed to help the SMN2 gene make more working protein. It is given into the spinal fluid once every 12 months. It enrolls people aged 15-60 who are new to treatment or have taken Evrysdi, with US sites in Palo Alto and Norfolk.
- PIERRE (NCT05866419) from Alcyone Therapeutics tests an implanted port and catheter for giving Spinraza. It enrolls people aged 3 and older for whom repeated lumbar punctures are hard, at 15 US sites. A linked Biogen sub-study, PIERRE-PK (NCT06555419), measures how nusinersen behaves when given through the port.
- The fifth study, at the University of Missouri, tests whole-body electrical muscle stimulation exercise in adults with several neuromuscular diseases including SMA (NCT07478172).
Each of these studies is testing an open question, and none of them promises a result. SOLAR's main results are estimated for June 2028 and STREAM's for May 2032, according to their registry records, and estimated dates often shift. Trial Friend's SMA trials page lists every recruiting study, and the SMA treatments comparison shows all approved options side by side.
Sam's timeline ends in a dashed line because the rest has not happened yet. When Sam was born, the gene behind SMA had only just been named. Now, 31 years later, Sam can bring a short list of questions about a one-time treatment to a neurologist with options to offer.
Itvisma Questions From Teens, Adults and Parents
Can adults get Itvisma for SMA?
Yes, the FDA approved Itvisma on November 24, 2025 for people with SMA aged 2 and older, including adults. Its trials enrolled only people aged 2-17, so adult coverage depends on your insurer's policy. Aetna and Cigna, 2 large insurers, cover it only through age 17.
Is Itvisma better than Spinraza for adults with SMA?
No one knows yet, because no trial has compared Itvisma with Spinraza in adults or in children. Both are injected into the spinal fluid, but Itvisma is a single dose while Spinraza continues every 4 months. STREAM is collecting adult Itvisma data in everyday US care now.
Do I have to stop Evrysdi or Spinraza before getting Itvisma?
Usually yes, because all 5 insurer policies Trial Friend read limit using them alongside Itvisma. Aetna and Cigna ask that Spinraza or Evrysdi be stopped before the dose. In the STRENGTH trial, people waited at least 4 months after their last Spinraza dose and at least 15 days after Evrysdi before receiving Itvisma.
Can I get Itvisma if I already had Zolgensma as a baby?
No, the Itvisma label says people previously treated with Zolgensma should not receive it, and every insurer policy Trial Friend read excludes prior SMA gene therapy. Children treated with Zolgensma may still use other SMA treatments, which their neurologist can discuss.
What are the main side effects and risks of Itvisma?
The label's boxed warning is for serious liver injury, which is why patients take prednisolone and have liver tests for at least 3 months. In the STRENGTH trial, low platelet counts were reported in 8 of 27 people and liver-related events in 4, with no deaths.
Does Medicare cover Itvisma?
Medicare has no national coverage decision for Itvisma, so each request is decided by the Medicare Advantage plan or by the regional contractor for original Medicare. Drugmaker copay programs such as Novartis's generally do not apply to Medicare, so ask Novartis Patient Support about other options.
