Five newer drugs are FDA-approved for hereditary angioedema, and 4 of them prevent attacks while 1 treats an attack once it starts. The preventives are Takhzyro, an injection every 2 weeks; Orladeyo, a daily capsule; Andembry, a monthly injection; and Dawnzera, an injection every 4 or 8 weeks, all taken at home. In their placebo-controlled trials they cut attack rates by 44% for Orladeyo and 81% to 89% for the 3 injections. Ekterly, approved July 2025, is the first oral on-demand treatment: 2 tablets at the first sign of an attack, with symptom relief in a median of 2 hours. People on a preventive still carry an on-demand medicine for breakthrough attacks.

What is the best preventive treatment for hereditary angioedema?

No trial has compared the 4 preventives. Against placebo, Andembry cut monthly attacks by 89%, Takhzyro by 87%, Dawnzera by 81% and Orladeyo by 44%, in separate trials with different designs. The practical choices are a daily capsule (Orladeyo) against an injection every 2 weeks (Takhzyro), 4 weeks (Andembry and Dawnzera) or 8 weeks (Dawnzera, in some patients), and which ages each covers.

Is there a pill to prevent hereditary angioedema attacks?

Yes. Orladeyo (berotralstat) is 1 capsule a day with food from age 12, with oral pellets for ages 2 to 11. In its trial it reduced attacks by 44% against placebo, less than the injected preventives, which were 81% to 89% in their own trials.

What is Ekterly used for?

Ekterly (sebetralstat) is taken when an attack starts: 2 tablets at the first sign, with a second dose 3 hours later if needed, for people 12 and older. In its trial, 76% reached symptom relief within 12 hours against 49% on placebo, with a median time to relief of 2 hours. It is the first oral on-demand HAE treatment and is not for prevention.

Which HAE preventive is given least often?

Dawnzera (donidalorsen), which is injected every 4 weeks and can be moved to every 8 weeks in patients who do well; Andembry is monthly. Takhzyro starts at every 2 weeks and can move to every 4 weeks after 6 attack-free months. Orladeyo is daily.
