Prescribing companion
Fabhalta (iptacopan) Fabhalta vaccine requirements, REMS, monitoring and prior authorization criteria for IgA nephropathyFor prescribers and their offices treating IgA nephropathy: what the pharmacy needs before it ships, what the prior authorization wants documented, the titration and the monitoring the label requires.
Dispensing gate
REMS-certified prescriber, pneumococcal and meningococcal (ACWY and B) vaccination at least 2 weeks before day 1 or antibacterial prophylaxis with vaccines as soon as possible, counseling with the Patient Safety Card, and the pharmacy's documentation of vaccination status with the office.
Example PA threshold
FEP example: biopsy, UPCR 1.5g/g or higher in a policy written before the July 2026 full approval removed that bar from the label, maximum ACEi or ARB dose, eGFR 20 or higher, vaccines and REMS enrollment.
Monitoring
Serious infection with encapsulated bacteria at any time, lipids periodically, boosters on the ACIP schedule for a complement inhibitor for as long as treatment lasts.
Support line
Novartis Patient Support, 1-833-993-2242, one named team member, Co-Pay Plus to $0 for commercial plans plus $1,000 a year toward vaccines, free drug for up to 12 months while a commercial plan decides, in-home vaccination; REMS helpline 1-866-201-3101.
Before the first fill: REMS, labs and enrollment
The steps that the pharmacy, the REMS or the drugmaker's program will wait on, in the order they usually need to happen. A first fill of a specialty drug most often stalls on one of these rather than on the plan.
| Step | What it requires | Source |
|---|---|---|
| Certify in the FABHALTA REMS | The REMS requires the prescriber to be enrolled, to counsel the patient on the infection risk, give the written materials, check vaccination status and hand over the Patient Safety Card. There is no patient enrollment form. | FDA REMS review |
| Vaccinate | The label requires vaccination against Streptococcus pneumoniae and Neisseria meningitidis serogroups A, C, W, Y and B at least 2 weeks before the first dose, on the ACIP schedule for complement inhibitor recipients. Prior standard-schedule vaccination may not meet it. The FDA removed the Hib requirement from the label in March 2024. | FDA label |
| Or prophylaxis | If treatment cannot wait 2 weeks, the label says to give antibacterial prophylaxis and the vaccines as soon as possible, and the REMS requires the prescriber to write that antibiotic prescription. | FDA REMS review |
| Expect the pharmacy's call | Before the first fill the certified pharmacy confirms the prescriber's certification and contacts the office to document vaccination status or the antibiotic plan. It keeps checking for up to 6 months if the patient was not up to date. The FDA's REMS review lets it dispense and keep following up rather than hold the drug. | FDA REMS review |
| Novartis Patient Support and the PA | Enrollment gives the patient one named team member, help locating and tracking vaccines, in-home vaccination at no cost for eligible patients, and for commercial plans free Fabhalta for up to 12 months while the authorization or appeal is pending, if filed within 45 days of enrolling. | Novartis Patient Support |
Prior authorization criteria and what to document
One public policy as the example: Blue Cross Blue Shield Federal Employee Program, April 1, 2026. Approval a year at a time, renewed if the urine protein ratio has fallen and the ACE inhibitor or ARB continues, with a limit of 400mg a day. This policy was reviewed in March 2026, before the FDA's July 2026 full approval removed the 1.5g/g protein bar from the label. A newer version may set a lower threshold. It also still lists the Hib vaccine, which the FDA dropped from the label in March 2024. It is here as an example of what a plan asks for, not as a rule.
| Element | What the policy asks | In the chart | Source |
|---|---|---|---|
| Diagnosis | IgA nephropathy confirmed by kidney biopsy, age 18 or older | Pathology report with date | FEP Blue policy |
| Proteinuria | UPCR 1.5g/g or higher in the March 2026 policy; the July 2026 full approval removed that bar from the label, so a newer version may set a lower threshold | UPCR result with date | FEP Blue policy |
| RAS blockade | Taken with the maximum recommended or tolerated dose of an ACE inhibitor or ARB, continued at renewal | Agent, dose and start date | FEP Blue policy |
| Kidney function | eGFR 20 or higher | Creatinine and eGFR with date | FEP Blue policy |
| Vaccination | Pneumococcal and meningococcal at least 2 weeks before starting unless treatment cannot be delayed; the policy still lists Hib, which the label dropped in 2024 | Vaccine names and dates, or the prophylaxis prescription | FEP Blue policy |
| REMS | The prescriber is enrolled in the Fabhalta REMS | Enrollment confirmation | FEP Blue policy |
| Prescriber | Prescribed by or on the advice of a nephrologist | Specialty on the request, or the consult note | FEP Blue policy |
Fabhalta dosing and titration
| When | Label | Source |
|---|---|---|
| Day 1 | 200mg twice daily, swallowed whole, with or without food; never opened, broken or chewed. A 60-capsule bottle is a 30-day supply. | FDA label |
| Missed dose | The Medication Guide says to take 1 dose as soon as remembered, even if the next dose is near, then resume the regular schedule. | FDA label |
| Stopping | The infection risk continues after the last dose; the Patient Safety Card is carried for 2 weeks after stopping. | FDA label |
Monitoring the Fabhalta label requires
| When | What | What the label says to do with it | Source |
|---|---|---|---|
| Before day 1 | Vaccination status against the complement inhibitor schedule | Required by the label; documented by the pharmacy with the office | FDA label |
| Any time | Serious infection: fever with or without chills, headache with nausea or stiff neck, fever with rash, confusion, tachypnea, photophobia, chest pain with cough, tachycardia, clammy skin, myalgia | Immediate medical care; in APPLAUSE-IgAN serious encapsulated bacterial infections occurred in 3 of 259 patients on Fabhalta and none on placebo | FDA label |
| Periodically | Total cholesterol, LDL and triglycerides | The label reports increases in the PNH trials and asks prescribers to check lipids and treat if needed; many offices run the first at 3 months with the protein and kidney function tests | FDA label |
| Per ACIP | Boosters | Revaccinate on the ACIP schedule for as long as treatment lasts; Novartis Patient Support tracks dates | FDA label |
| First weeks | Abdominal pain, dizziness, nausea | 8% against 4%, 5% against 2%, 4% against 1% in APPLAUSE-IgAN, all mild or moderate | FDA label |
Fabhalta drug interactions and contraindications
| Label category | Agents and what the label says | Source |
|---|---|---|
| Not recommended | Strong CYP2C8 inhibitors such as gemfibrozil | FDA label |
| Avoid | CYP2C8 inducers such as rifampin, which can reduce iptacopan exposure | FDA label |
| Hepatic impairment | Not recommended in severe hepatic impairment | FDA label |
| Pregnancy | Data insufficient to establish risk; the label leaves use to a benefit and risk assessment | FDA label |
Why a first fill stalls, and the fix
- The pharmacy cannot document the vaccinesThe REMS requires the pharmacy to confirm your vaccination status or the antibiotic plan with the prescriber before the first fill. The fix is the office sending the vaccine dates, or the prescriber writing the antibiotic prescription. The FDA's REMS rules let the pharmacy dispense and keep following up rather than hold the drug, so a long delay here is worth a call.
- The prescription went to a pharmacy outside the networkOnly REMS-certified pharmacies dispense Fabhalta, through the small network Novartis uses. The prescriber's office resends it. This is a routing problem, not a denial.
- Prior authorization pending or deniedPlans ask for the criteria below, most often the biopsy, a protein measurement, an ACE inhibitor or ARB at full dose, and proof of the vaccines and REMS enrollment. For commercial insurance the Novartis bridge supplies Fabhalta free for up to 12 months while the authorization or appeal is pending, if it is filed within 45 days of enrolling.
- The vaccines themselves cost moneyNovartis's copay program covers up to $1,000 a year of vaccine costs for commercial insurance. Its Vaccination Support program gives the shots at home at no cost for eligible patients regardless of insurance.
- A copay you cannot payCommercial insurance: the Co-Pay Plus program, as little as $0 up to $20,000 a year. Medicare, Medicaid, TRICARE and VA: the program's terms exclude you, and Novartis points to its Patient Assistance Foundation at 1-800-277-2254. Our help paying page lists the charity funds that are open.

Questions prescribers' offices ask about Fabhalta
Which vaccines are required before Fabhalta?
Streptococcus pneumoniae and Neisseria meningitidis serogroups A, C, W, Y and B, at least 2 weeks before the first dose, on the ACIP schedule for complement inhibitor recipients. The FDA removed the Hib requirement from the label in March 2024.
Can Fabhalta start before the vaccines have had 2 weeks?
Yes, if the prescriber decides treatment cannot wait. The label says to give antibacterial prophylaxis and the vaccines as soon as possible, and the REMS requires the prescriber to write that antibiotic prescription.
What does the Fabhalta REMS require?
A certified prescriber who counsels the patient, gives the written materials, checks vaccination status and hands over the Patient Safety Card. There is no patient enrollment form. The certified pharmacy documents vaccination status with the office before the first fill. The REMS helpline is 1-866-201-3101.
What monitoring does the Fabhalta label require?
Vigilance for serious infection with encapsulated bacteria at any time, periodic lipid checks because cholesterol and triglycerides can rise, and boosters on the ACIP schedule for as long as treatment lasts.
What do insurers ask for to approve Fabhalta for IgA nephropathy?
The FEP policy shown on this page, dated April 1, 2026, asks for biopsy-confirmed IgA nephropathy, UPCR of 1.5g/g or higher, the maximum ACEi or ARB dose, eGFR of 20 or higher, the vaccines, REMS enrollment and a nephrologist. It predates the July 2026 full approval, which removed the 1.5g/g bar from the label.
