The FDA is expected to decide on Vyglxia for Spinocerebellar ataxia in Q1 2027, under an NDA resubmission from Biohaven.
What is being decided
Biohaven has an NDA resubmission under FDA review for Vyglxia (troriluzole) in Spinocerebellar ataxia. Troriluzole is a once-daily pill that the body turns into riluzole, an older medicine for ALS. It is being studied in spinocerebellar ataxia (SCA), an inherited disease with no FDA-approved treatment that slows it. On November 4, 2025, the FDA declined to approve Biohaven's first full application. The FDA's letter said the main evidence did not meet its standard for a well-controlled study. That evidence compared people on troriluzole for 3 years with people in a separate study who were not taking it. The 48-week part of the trial that compared the drug with a placebo, a dummy pill, did not meet its main goal. On October 22, 2026, Biohaven said the FDA had accepted a second application with what the company called new positive supportive analyses. The FDA plans to decide in the first quarter of 2027 and to hold a public meeting of outside experts, with no date set yet. Accepting an application for review does not mean the drug will be approved. ClinicalTrials.gov still lists troriluzole's expanded access program (NCT06034886) as available, but an ataxia doctor told a Senate committee in February 2026 that the program was about to end. Ask a neurologist to check with Biohaven whether it is still taking patients.
Who this decision matters to
Spinocerebellar ataxias are a group of inherited neurodegenerative disorders caused by mutations in various genes, leading to progressive degeneration of the cerebellum and its connections. There are over 40 identified genetic subtypes, each with distinct genetic causes but overlapping symptoms of progressive incoordination, balance problems, and speech difficulties.
Prevalence: Collectively about 1-5 per 100,000 people; SCA3 (Machado-Joseph disease) is the most common subtype worldwide. See our full Spinocerebellar Ataxia page for current treatments, recruiting trials, and community resources.
About the drug
Vyglxia: the body turns troriluzole into riluzole, which is thought to calm overactive nerve signals by lowering glutamate, a chemical messenger. That may help protect nerve cells in the cerebellum, the part of the brain that controls balance and coordination. Our full Vyglxia profile covers how it works, side effects in plain language, and its trial history.
What each outcome would mean
An approval starts a second race rather than ending the first one: specialty pharmacy setup, insurance review, and patient assistance typically take weeks even when everything goes right. Our guide to the 90 days after a rare disease approval explains the timeline and the moves families can make on day 1.
A complete response letter would mean the FDA declined to approve in the application's current form. The letter describes what the FDA wants addressed, and the company then decides whether to resubmit, run more studies or stop. Either way, this page updates with the outcome and what it means.

Research into SCA is advancing on multiple fronts, with considerable focus on halting the underlying disease mechanisms. Biohaven's troriluzole missed its main goal in the placebo-controlled part of its Phase 3 study.
Biohaven then compared patients who stayed on the drug for 3 years with patients from separate natural history studies who were not taking it, and reported 50% to 70% slower worsening.
On November 4, 2025 the FDA issued a Complete Response Letter citing concerns with that outside comparison, and recommended a meeting to discuss the evidence a future application would need. Biohaven then announced plans to cut its yearly research spending by about 60%, and later said speeding up other trials would make that cut smaller.
Biohaven said on October 22, 2026 that the FDA accepted a resubmitted application for review, with a decision expected in the first quarter of 2027. Accepting the application for review does not mean the drug will be approved.
ClinicalTrials.gov still lists troriluzole's expanded access program (NCT06034886) as available, with the record last updated January 9, 2026. An ataxia specialist who treats patients in the program told a Senate committee in February 2026 that it was about to end, so ask a neurologist to confirm with Biohaven whether it is still taking patients.
Other active areas include antisense oligonucleotides designed to silence specific mutated SCA genes and broader neuroprotective strategies. Different SCA subtypes may require different therapeutic approaches, which is why genetic testing to confirm your specific subtype is so valuable.
The National Ataxia Foundation maintains a comprehensive trial registry and offers strong patient support. Work with your neurologist to understand which trials match your specific SCA type.

When will the FDA decide on Vyglxia?
Biohaven expects a decision in Q1 2027 but has not given an exact date.
Who disclosed it: Biohaven. The FDA does not publish these dates.
Can it move: the FDA can act early, and a major amendment to the application can push the date back 3 months.

Is there already an FDA-approved treatment for Spinocerebellar Ataxia?
No. No medicine's current FDA label names Spinocerebellar Ataxia, checked October 21, 2026.
Check: the Spinocerebellar Ataxia page also searches FDA drug labels for approved treatments.
What did the Vyglxia trials show?
Biohaven's Phase 3 study (Study 206) began with a 48-week placebo-controlled phase in 218 people. It missed its main goal, the change on the f-SARA scale at week 48.
More: the Vyglxia drug page has the full trial history and sources.
What are the side effects of Vyglxia?
In the 48-week placebo-controlled part of the Phase 3 study, the most common side effects in people taking troriluzole were headache (12%, against 7% on placebo), tiredness (9% against 8%) and nausea (6% against 1%). Dizziness was reported slightly less often on troriluzole than on placebo (8% against 10%).

Can I get Vyglxia before the FDA decides?
Usually only through a clinical trial or an expanded access program, and the company decides whether to offer one.
Trials: 27 Spinocerebellar Ataxia studies are open or about to open, 18 of them testing a treatment. See them.
Check your fit: Match Me compares your details with every open trial.
Expanded access: your doctor can ask Biohaven directly. Registered programs are listed on ClinicalTrials.gov.
How much will Vyglxia cost?
Biohaven has not announced a price. Drugmakers usually set it around approval.
Help paying: drugmaker programs and charity funds usually open at launch.
Find help: the patient assistance finder lists programs by insurance type.
What happens after the Vyglxia decision?
This page updates with the outcome the day it is announced.
If approved: getting the drug still takes time. Specialty pharmacy setup, insurance review and patient assistance often take weeks. Our guide covers what families can do on day 1.
If not approved: a complete response letter means the FDA declined the application in its current form. Many are about manufacturing or how data were presented, and companies often resubmit.
